darcs patch: Fix Haddock errors. (and 22 more)

The attached patches fix haddock parse errors for GHC.  Expect for one or
two cases I did not add existing comments to haddock but only tried to
remove parse errors.

So, from now on be careful that you don't break haddocking by using
*emphasis* (use _emphasis_ or /emphasis/ instead), pointers (^^) to 
parts of the source code or empty "where" clauses.  I will put up a
wiki page with the types of errors I've found (some are tricky).
