    Final batch of monad-comprehension stuff
    
    * Do-notation in arrows is marked with HsStmtContext = ArrowExpr
    * tcMDoStmt (which was only used for arrows) is moved
      to TcArrows, and renamed tcArrDoStmt
    * Improved documentation in the user manual
    * Lots of other minor changes
