[xmonad] XMonad.Operations.windows oddities

1) Is the unconditional restackWindows in XMonad.Operations.windows really
necessary?  That's the root cause of the oddball window reorderings when
floating windows are given focus.  If a floating window has focus I would
simply move it to the top of the z-order (raiseWindow), not reorder the
whole stack in what amounts to an arbitrary way.

2) I find myself wondering if the fact that the border size is added to a
window's rectangle *before* scaleRationalRect is applied to it is related to
the occasionally observed phenomenon of floating windows being resized upon
gaining focus.  Certainly it is sketchy.
